Our broad, long-term objective is to improve outcomes after a burn injury by utilizing an expanded care coordinator (ECC) to assist with the management of barriers to effective burn rehabilitation. Our specific aim is to determine if the ECC intervention will improve outcomes across multiple domains for adults with burn injuries. We hypothesize that adults with burn injuries receiving the ECC intervention will be more successful at attaining their rehabilitative outcome goals, and achieve higher levels of health and function, participation and community re-entry than those receiving standard outpatient care.
